Join the Early Supported Discharge Service based on Sapphire Ward at Gravesham Community Hospital, supporting patients recovering from stroke and neurological conditions as they transition from hospital to home. You will deliver high-quality, patient-centred rehabilitation, helping people regain independence and confidence in their daily lives. The role is varied, combining inpatient work with community visits so you can follow patients through their recovery journey, working within a dynamic and supportive multidisciplinary team that values excellent care and continuous service improvement. The post is full time across inpatient and community settings and requires a full UK driving licence and access to a vehicle. Closing date: 26 July 2026.
